Partnering for Impact on Behavioral Health in the ED

A new collaboration spearheaded by WakeMed Health & Hospitals was announced this past week. Through partnership with Blaze Advisors, a specialist in developing Accountable Communities of Health, the Network for Advancing Behavioral Health (NABH) combines coordinated care management protocols, systems, and quality improvement initiatives that impact a multi-county, cross-disability populations.

 

The goal of this project is to assure that patients challenged with primary or secondary behavioral health and/or substance use disorders receive the right care at the right time; rationalizing resource utilization and improving patient satisfaction. Developed to systematically involve community-based social service and behavioral health providers, WakeMed patients are linked with high-quality care from specialists (i.e. psychiatrists, counselors, substance use professionals) as well as benefit from streamlined access to community services (i.e. housing, food, transportation) that mitigate social determinant obstacles to care.
